JOB SUMMARY
This position will serve as the family care navigator for the Division of Substance Use Prevention and Harm Reduction’s (SUPHR) Philly LIFTS (Linking Infants and Families to Service) program. Philly LIFTs is an outreach program for the families of infants diagnosed with Neonatal Abstinence Syndrome (NAS), along with families affected by substance use disorder at all stages. Philly LIFTS is part of SUPHR’s NAS Program, which conducts surveillance of NAS diagnoses in Philadelphia. The primary goal of the program is to support the families of infants diagnosed with NAS, along with families affected by substance use disorder. The program aims to “lift” parents and their children to support and services. This support includes, but is not limited to food referrals, baby supplies like diapers and car seats, clothing, employment assistance, referral to home visiting programs and doula support, recovery support, short-term therapy sessions for children 5-21 years old, mental health support for postpartum parents, and overdose prevention training. Philly LIFTs hosts community outreach events and celebrations throughout the year. These events are open to all families, not only those enrolled in the program.
The Family Care Navigator will work in-tandem with the Senior Family Care Navigator to connect families impacted by substance use to needed clinical and social services. This individual will be responsible for supporting the continuity of care for families served by the program and will be situated within the Philadelphia Department of Public Health.
JOB SPECIFICATIONS
Responsibilities/Duties
• Assist families impacted by substance use by connecting them to needed clinical and social services
• Build and maintain relationships with the families served by the program
• Maintain a streamlined referral process that includes checkpoints for verification of successful connection to services
• Cultivate relationships with substance use treatment providers, specifically those targeting families
• Support the coordination and hosting of social groups and gatherings with patients of substance use treatment centers
• Identify agencies providing social services and establish partnerships with those agencies with a focus on agencies providing services to perinatal populations and families
• Identify agencies and establish relationships with agencies providing behavioral and developmental services, including those providing services to children age 0 to 5 yrs.
• Collect and maintain data surrounding family and community outreach metrics
• Attend community events and participate in relevant councils and committees as a Philly LIFTS representative
• Other duties as assigned
Education:
• Obtained high school diploma or GED
• Certified as a peer (CPS) or recovery specialist (CRS), or willingness to obtain certification upon start of position.
• Certified as family recovery specialist (CFRS), if ineligible for CPS and CRS certifications
Skills/Experience
• At least 2 years of experience in case management, peer navigation, or linkage to substance use disorder care services
• Lived experience in substance use (broadly defined – e.g., personal, familial, community), preferred
• Knowledge of harm reduction, trauma-informed care, stigma reduction, and substance use, preferred
